The post holder will support the trained nurse and other health care professionals in the delivery of the highest standard of patient focused care.
Improve the Quality of the fundamental aspects of Health and Social Care for people who are acutely or chronically ill, frail or disabled.
All HCSW posts will be recruited on the basis that they will complete Level 2 NVQ within 30 months of appointment. It is a requirement of the individual that they must have completed an NVQ to the required level before a permanent support workers post will be offered by the Health Board.
Ensure that the 12 fundamentals of care are applied in the care of all patients: Communication and information, respecting people, ensuring safety, promote independence, relationships, sleep, rest and Activity, ensure comfort, alleviate pain, personal hygiene and appearance, eating and drinking, toilet needs, oral health and hygiene and preventing pressure sores.
Assistance and support to the patient in maintaining an acceptable quality of life and performing tasks, or helping them to perform tasks that they would normally do for themselves or that would be done by a close relative.
Undertaking direct patient care tasks under the direct and indirect supervision of a Registered Nurse.
Assist the Registered Nurse or other team member in carrying out nursing procedures or activities that require the presence of two people.
Undertaking duties directly involved with patient care and the maintenance of the patient care environment.
To report to the Registered Nurse or Team leader any changes in condition or behaviour of the patient.
To display sensitivity to the individual needs of each patient whilst preserving dignity.
Respect and maintain patient and colleague confidentiality.
To participate in and co-operate with the nursing team and other members of the multi-disciplinary team to contribute to the effectiveness of the team as a whole.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
