JOB PURPOSE The Health and Wellbeing Coach will work closely with the Primary Care team to help people who need support to develop the knowledge, skills, and confidence to manage their health and wellbeing. The role will: - Be part of a network-based Wellbeing Team including: Social Prescribers, Heath Coaches, Care Co-ordinators and Dieticians offering a whole person approach to address lifestyle issues and encourage early prevention of ill health and better self-management of existing conditions. Involve proactive work across communities with partners where there are known levels of high deprivation associated with poor health outcomes. Work in partnership with practice clinicians and other team members who identify individuals for whom non-medical intervention could improve the quality of their life for example living with a long-term condition/s, chronic pain management, the impact of adverse life experiences or bereavement.

Use health coaching skills to build motivation and confidence in individuals to take action to support themselves and to utilise or build their community support network. This will be through a mix of 121 support and group work base on needs. Take a non-judgemental approach, based on strong communication and negotiation skills that supports personal choice and ensure patients understand the accountability of their own decisions. Main Responsibilities: Coach and motivate patients individually through 121 sessions to identify their needs and implement change, or through a group-based approach delivered with colleagues and wider partners that target specific cohorts identified via practice or public health data.

Provide support to individuals, their families and carers to ensure they are active participants in their own healthcare; empowering them to take more control in managing their own health and wellbeing, to live independently, and improve their health outcomes through: Providing interventions such as self-management education, enabling positive lifestyle intervention and access to peer support. Supporting people to establish and attain goals based on what is important to them for their health and wellbeing. Working with Social Prescribers, Public Health community teams and wider partners to co-deliver community-based projects that are uniquely developed for an identified cohort and its individual needs. Ensure that fellow PCN staff are made aware of Health and Wellbeing Coaching and Social Prescribing services.

Support colleagues within General Practice and wider to improve their skills and understanding of personalised care, shared decision making and behavioural approaches and ensuring consistency in the follow up of peoples goals where an MDT is involved. To work with people with lower activation to understand their level of knowledge, skills, and confidence (their Activation level) when engaging with their health and wellbeing. Utilise existing IT and MDT channels to screen patients, with an aim to identify those that would benefit from Health and Wellbeing Coaching. * A full Job Description & Person Specification is attached to this advert*
